You can use this tool to get a snapshot of how your pet is doing right now or use it regularly to track trends and see how they are responding to care.
Please select the answers that best reflect your pet's experience. There are no right or wrong answers — this is simply a tool to help you notice what they are telling you through their behaviour.
Once you've completed the assessment, you can simply view your results or choose to have them emailed to you so you can keep a record or review them again later. If you would like further support or would like to discuss your results, you may wish to speak with your regular veterinarian or contact My Companion Mobile Vet to arrange an in-home quality-of-life assessment or discuss palliative care and end-of-life care, including peaceful in-home euthanasia when the time is right.

A Note on Tracking Quality of life can fluctuate. Keeping a simple daily record, such as marking good and bad days on a calendar, can help you and your veterinarian identify patterns over time and monitor how your pet responds to new treatments. Note: If your pet is currently recovering from a known temporary illness or surgery, this score may reflect a temporary dip rather than a permanent decline. Please consult your vet if you are unsure.
Disclaimer: This tool is intended for educational and guidance purposes only. It is not a substitute for professional veterinary advice, diagnosis, or treatment. Quality of life is complex and subjective; your veterinarian is the best person to help you interpret these results in the context of your pet's specific medical history and individual needs.
